MCKESSON PRIVACY POLICY
Welcome to Blue Health Connection® ("BHC"). BHC is a health information tool, product, and service provided by the Blue Cross and
Blue Shield Association ("BCBSA") that permits Service Benefit Plan Members to more fully utilize some of the services that are available
through the Service Benefit Plan. BHC functionality is being supported by McKesson Health Solutions LLC ("McKesson"), which is a Business
Associate of BCBSA, and McKesson subcontractors, including but not limited to HealthTrio, LLC and Health Media, Inc. ("McKesson Subcontractors").
The BHC health information tools supported by McKesson consist of website and internet-based services offering a secure, private channel for personal health record ("PHR")
storage, member benefit statement ("Benefit Statement"), and health risk assessment ("HRA"), on-line health care information, patient education, and
related products, services and content (collectively the "BHC Services"). You can tell when you are on a McKesson-supported health information tool because
the bottom of the webpage will be labeled "Powered by McKesson." If you are on a McKesson Subcontractor supported tool, the bottom of the webpage will be
labeled by the respective McKesson Subcontractor.
These BHC TERMS AND CONDITIONS OF USE describe your responsibilities when using this website. The
McKesson Privacy Policy describes how the information you provide through this website might be used
and disclosed by McKesson and McKesson Subcontractors. In general, a reference to "McKesson" in either
the BHC Terms and Conditions of Use or in the McKesson Privacy Policy also shall include "McKesson Subcontractors"
unless another meaning is expressly noted. And, when McKesson and McKesson Subcontractors both are referenced,
such dual references generally will be for purposes of enhancing clarity or for emphasis. The McKesson Privacy
Policy is distinct from BCBSA's NOTICE OF PRIVACY PRACTICES
http://www.fepblue.org/about/privacy.html, which
describes in detail how we (BCBSA and McKesson as BCBSA's Business Associate) handle individually identifiable health
information that we may collect both online and offline to provide services to you and to manage health care operations.
BCBSA publishes the NOTICE OF PRIVACY PRACTICES in accordance with the HIPAA Privacy Rule (a federal regulation officially
known as the "Standards for the Privacy of Individually Identifiable Health Information"). Together, the NOTICE OF PRIVACY
PRACTICES and the McKesson Privacy Policy describe how McKesson and McKesson Subcontractors may use and disclose your personal
information received both online and offline.

A. TERMS AND CONDITIONS OF USE
BHC Services are offered to you under, and you agree to be bound by, the terms and
conditions of this Terms and Conditions of Use. The BHC Services are available only to
members of the Blue Cross and Blue Shield Service Benefit Plan. We reserve the right to update or
change these Terms and Conditions of Use at any time and for any reason by posting the modified Terms
and Conditions of Use. Your continued use constitutes your agreement to be bound by any such revisions
and you should therefore periodically visit this page of the website and print the latest version of
the Terms and Conditions of Use for your records. The date of the last update to these Terms and
Conditions of Use is stated at the top of this document.
Please read these Terms carefully. By clicking "Accept" and accessing or using
BHC Services and/or this website, you agree to be bound by the terms and conditions of this Agreement.
If you do not wish to be bound by these Terms and Conditions of Use, you are not authorized to access or
use BHC Services and/or any of its components.
Use by Children. BCBSA and McKesson are committed to protecting the privacy of children. This
website and BHC Services are designed and intended for use by adults, and are not intended for, nor designed to
be used by, children under the age of eighteen. A parent or guardian, however, may use the BHC Services on
behalf of a minor child. The parent or guardian assumes full responsibility for ensuring that the authentication
information is kept secure and that the information submitted is accurate. The parent or guardian also assumes
full responsibility for the interpretation and use of any information or suggestions provided through BHC Services
for the minor.
B. USE OF SERVICES
NOT FOR EMERGENCIES. BHC Services are not for use for medical emergencies or urgent
situations. IF YOU THINK YOU MAY HAVE A MEDICAL EMERGENCY, CALL YOUR DOCTOR OR 911 IMMEDIATELY.
Medical Disclaimers. Nothing contained in this website, in the BHC Services, or in these
Terms and Conditions of Use should be construed, directly or indirectly, as medical advice, instructional for
medical diagnosis or treatment, or the practice of medicine or dispensing of medical services by BCBSA or McKesson.
Any health information furnished on this website is not intended to be complete, to suggest or recommend a course of
treatment, or to replace personal consultation with a qualified health care professional. Always seek the advice of
your physician or other qualified health provider with any questions you may have regarding a health or medical
condition. Never disregard, avoid or delay obtaining professional medical advice from your doctor or other
qualified health provider because of something you have read on this website! BCBSA and McKesson do not
guarantee that a web-based consultation (i.e., through Ask-a-Nurse or Live Chat) is the appropriate course
of treatment for your particular health care problem. You agree to contact your Provider or your local
emergency services immediately if you are experiencing an emergent or urgent condition.
Your Account. You represent and warrant that you will register on the BHC Services website under
your own name or as authorized by a covered family member and to use the BHC Services in accordance with these Terms
and Conditions of Use and abide by the obligations hereunder. You are solely responsible for maintaining the confidentiality
of your password, and for all activities that occur under your password. You agree to keep your password confidential
and immediately notify McKesson (at 1-888-258-3432) of any unauthorized use of your password or other security concerns
of which you become aware. Each person 18 and older covered under a Service Benefit Plan contract will be able to
obtain a user name and password by registering to use BHC.
Personal Health Record and Benefit Statement. Your PHR and Benefit Statement are created to store certain
personal health information online while a Service Benefit Plan Member, including health conditions, allergies, medications
and health services received such as provider or hospital visits. You agree to provide accurate information for your PHR,
to periodically review such information and to update such information as needed. You may provide only information that
you own or have the right to use. Additionally, your PHR and Benefit Statement contains certain information from your Service
Benefit Plan claims records. If any of your Service Benefit Plan claims information is incorrect, you will need to contact the
customer service representative at your local Blue Cross and/or Blue Shield Plan. For additional information regarding BCBSA's
and/or McKesson's use of the individually identifiable health information in your PHR, Benefit Statement and HRA, please see the
BCBSA Notice of Privacy Practices http://www.fepblue.org/about/privacy.html.
You may give permission to another person ("Authorized Person"), such as a family member, to access and manage
your PHR and Benefit Statement and such access shall be granted upon designation by you. Your permissions to access your PHR or Benefit
Statement may be terminated or revised at any time by changing the permission granted in your "Personal Profile". You may choose to give
the Authorized Person full access to your record, or you may choose to limit the information they can see. It is your responsibility to confirm
any information in your PHR that you permit an Authorized Person to enter.
Secure Message Center Communication. Your BHC Services account will allow you to post
non-emergent/urgent health-related questions to health care professionals in a Secure Message Center. In
an effort to achieve timely and private communication with you, you will be notified of pending messages
in the Secure Message Center through an e-mail sent to the e-mail address you provided during registration
on the BHC website that you have mail in the Secure Message Center.
Educational Resources. The information you provide during registration may be used to send you personal e-mails or electronic
messages, including information regarding common medical and health related topics or preventive care messages containing specific medical and health
related information, links to other related websites and specific questions or information related to the BHC website. Additionally, there may be
self care informational services which provide general medical and health information. These communications and resources are not comprehensive medical
text and do not include all the potential information regarding the subject matter. These communications and resources are for general educational and
informational purposes only, and should not be relied upon as a substitute for patient-specific medical diagnosis and treatment or health care consultation
or construed, directly or indirectly, as the practice of medicine or dispensing of medical services. Such information is not a substitute for seeing an
appropriate health care professional for medical treatment. The information contained in these communications and resources is compiled from a variety
of sources, including from McKesson Subcontractors and from certain third parties and should not be considered authored by BCBSA or McKesson. Neither BCBSA,
McKesson nor McKesson Subcontractors make any warranty as to the content of these materials or the information contained therein.
Opt Out. If you make a request to receive information, including Secure Messaging Notification e-mails,
in an ongoing manner through this website by providing your e-mail address you may make a request to discontinue any future mailings.
Also, if you receive information about BHC through e-mail, you may make a request to discontinue receiving such messages in the future.
Information sent to you by e-mail will contain information about how you may opt out of receiving such e-mails. You may also register
to use certain protected features on the BHC website, which may provide you an opportunity to receive e-mails about different types of
BHC products, services, announcements, and updates.
If after receiving such e-mails or electronic messages, you prefer not to receive them in the future, you
may opt out of these communications by indicating your preference on your "My Profile" page under My Preferences.
Intellectual Property. BCBSA and/or McKesson (as applicable) hereby grant you a limited, revocable, non-transferable
and non-exclusive license limited solely to viewing or downloading a single copy of the material on the BHC website and to use the BHC
software to the extent necessary to use the BHC Services solely for you personal, noncommercial use. With the exception of your PHR
(but not the underlying technology), BCBSA, McKesson, or McKesson Subcontractors (as applicable) retain all right, title and interest
in and to BHC, the BHC Services and any content, products, documentation, software or other materials on the BHC website, and any patent,
copyright, trade secret, trademark, service mark or other intellectual property or proprietary right in any of the foregoing. The
information available through the BHC Services is the property of BCBSA and/or McKesson (as applicable) or McKesson Subcontractors and
is protected by United States copyright, trademark, and other intellectual property laws. You agree not to reproduce, retransmit,
distribute, disseminate, sell, publish, broadcast, or circulate the information owned by BCBSA and/or McKesson (as applicable) or their
licensors received through the BHC Services to anyone, including but not limited to others in your organization. Any copy made of information
obtained through the BHC site must include the copyright notice. Use, reproduction, copying, or redistribution of McKesson's, BCBSA's,
BHC's, or McKesson Subcontractors' logos is strictly prohibited without written permission. For additional information concerning
McKesson's and/or BCBSA's (as applicable) intellectual property rights, see Intellectual Property under the Legal Notices section of this

BCBSA and McKesson respect your privacy and take privacy very seriously. By accepting this Terms and
Conditions of Use, you consent to the uses and disclosures of individually identifiable health information
provided to us as outlined in BCBSA's Notice of Privacy Practices (which is incorporated herein by reference)
and as outlined below in McKesson's Privacy Policy.
E. GENERAL LEGAL PROVISIONS
Third Party Sites. Although this website may include links providing direct access to third-party
Internet sites as a convenience, the inclusion of a link does not imply endorsement of the linked site by
BCBSA or McKesson or McKesson Subcontractors. BCBSA and McKesson take no responsibility for the content or
information contained on those other sites, and does not exert any editorial or other control over those
other sites. Nor do BCBSA and McKesson take responsibility for the privacy policies and practices of
these third-party links.
Operation and Record Retention. BCBSA reserves complete and sole discretion with respect to
the operation of the BHC Services. BCBSA may, among other things withdraw, suspend or discontinue any functionality
or feature of the BHC Services. BCBSA and McKesson are not responsible for transmission errors or corruption or
compromise of data carried over local or interchange telecommunication carriers. BCBSA and McKesson are not
responsible for maintaining data arising from use of the BHC Services. BCBSA and/or McKesson reserve
the right to maintain, delete or destroy all communications and materials posted or uploaded to the BHC
website pursuant to BCBSA's internal record retention and/or destruction policies.
Limitations on Use. You agree not to access or use the BHC Services in an unlawful
way or for any unlawful purpose. You shall send e-mails to other parties solely for personal communication
and no other purposes, including commercial purposes, or for mass distributions of unsolicited e-mails.
You agree not to post or transmit: (a) a message under a false name; or (b) any information which (i) is
libelous, defamatory, obscene, fraudulent, false or contrary to the ownership or intellectual property
rights of any other person, or (ii) contains any virus, worm, Trojan horse or other code which is contaminating
or destructive to the files or programs of BHC or any of its users. BCBSA and/or McKesson reserve the right to
delete any information provided by you that it deems in its sole discretion fraudulent, abusive, defamatory,
obscene or in violation of a copyright, trademark or other intellectual property or ownership right of any
other person.
Third Party Rights. The provisions under Section C Legal Notices and Disclaimers are for the
benefit of BCBSA, McKesson, McKesson Subcontractors and their affiliates. These individuals or entities shall have
the right to assert and enforce those provisions directly against you on its own behalf.
Term and Termination. These Terms and Conditions of Use shall commence upon the date
you first access or use the BHC Services and/or complete the registration process. Either you or BCBSA may
terminate these Terms and Conditions of Use and your right to use the BHC Services at any time, with or without cause.
These Terms and Conditions of Use and the licenses granted hereunder shall terminate without notice in the event you
(or any Authorized Person using your account) fail to comply with the terms and conditions of these Terms and Conditions of
Use, or the rules for use of the BHC Service promulgated by BCBSA and/or McKesson from time to time.
Notice. Any notices regarding the BHC website may be provided to you by e-mail to the
e-mail address you provided during registration for BHC Services, by a general notice on the BHC website,
or by written communication delivered by first class U.S. mail or express courier to your address on record
in the BHC Services account information.
Severability. The provisions of these Terms and Conditions of Use are severable, and
in the event any provision hereof is determined to be invalid or unenforceable, such invalidity or unenforceability
shall not affect the validity or enforceability of the remaining provisions, but such provision shall be reformed,
if reasonably possible, only to the extent necessary to make it enforceable.
Entire Agreement. These Terms and Conditions of Use, together with any McKesson,
BCBSA, or BHC rules or policies referred to herein, represents the complete agreement between you and
BCBSA/McKesson concerning the subject matter hereof, and it replaces all prior oral or written communications
concerning such subject matter. BCBSA and/or McKesson may modify these Terms and Conditions of Use as set
forth above.
Assignment. You may not assign, transfer or delegate these Terms and Conditions of
Use or any part of it without prior written consent. BCBSA and/or McKesson may freely transfer, assign
or delegate all or any part of these Terms and Conditions of Use, and any rights and duties hereunder.
These Terms and Conditions of Use will be binding upon and inure to the benefit of the heirs, successors
and permitted assignees of the parties.
Waiver. Failure to exercise or delay in exercising any right hereunder, or failure
to insist upon or enforce strict performance of any provision of these Terms and Conditions of Use, shall
not be considered waiver thereof, which can only be made by signed writing. No single waiver shall be
considered a continuing or permanent waiver.
Governing Law. These Terms and Conditions of Use shall be governed by and construed
in accordance with the laws of the State of Illinois without giving effect to any choice of law rules or
principles. Any cause of action or claim you may have with respect to BHC Services must be commenced
within one (1) year after it arises, except to the extent such limitation is not enforceable. Any
dispute or claim in connection with this BHC website or your use of it will be brought in the State
or federal courts of Illinois. You agree to jurisdiction and venue in such courts. To the fullest
extent permitted by law, each party to this Terms and Conditions of Use waives its or his or her right to a
jury trial with respect to any dispute or other controversy arising from hereunder or your use of or access
to the BHC Services or website.
Other Terms. By using this BHC website, you are agreeing to the terms and conditions
of the paragraphs above and this paragraph. As above, these terms may be changed from time to time. So,
please check them frequently, since they govern your use of the BHC Services and BHC website.

INFORMATION COLLECTED
The kind of information we collect about you depends greatly on how you choose to use our site. To take
advantage of our online services, you will need to fill out our membership registration, which asks for
certain personally-identifiable information. In particular, we may ask for your health plan member ID number,
home address, date of birth, gender, telephone number and E-mail address. All this information makes up your
"Personal Profile," which will be associated with the username and password you choose. In addition to the
information you choose to share with us, our system is set up to gather certain technical data to help us
understand better how our site is being used and how we can improve it. This automatically gathered information
is not personally identifying; it includes your computer's IP or "Internet Protocol" address, statistics about
how visitors navigate through our site, and information provided through the use of cookies (which are
described in more detail below). Finally, as you visit our site, please be aware that we may link to
third parties' sites and these sites may at times gather information from you. In these cases, the
third parties would provide you with their policies and disclaimers and they would use your information
according to these policies. See "Your Options" below for more information about what you can do to
limit third party access to your information.
USE OF INFORMATION
It is important for you to know how your information will be used before you decide what to disclose. We
will not use your information for purposes other than these unless you give us your permission or we are
legally compelled to do so, such as by a subpoena or court order.
General Health Information:
If you access general health information or general health care reference materials, we will share your
personal health information only with BCBSA and not with any third party, or your employer unless permitted
or required by law or regulation. Please note that we may share your information with McKesson Subcontractors,
but we have written agreements with them that forbid them from using your personal information for their own
purposes and that restrict their uses and disclosures of such personal information to those same uses and
disclosures for which McKesson is permitted to use and disclose your personal information.
Use of Collected Information:
We will tailor the material and follow-up communications we provide you based on the information in your profile,
the Health Risk Assessment and other tools, and enrollments and programs. Follow-up communications may include contact
with McKesson to provide you with appropriate clinical care management and coaching. We may also collect information
about your use of our products and services and use that information to 1) respond to requests for information from BCBSA;
2) provide you with information regarding BHC Services; and 3) respond to your inquiries regarding BHC Services. Please
note that we may share your information with McKesson Subcontractors, but we have written agreements with them that
forbid them from using your personal information for their own purposes and that restrict their uses and disclosures of such
personal information to those same uses and disclosures for which McKesson is permitted to use and disclose your personal
information.
Personal Health Information:
If you choose to create a personal health record (PHR), we will share your personal health information only with BCBSA
and not with a third party unless you give us permission or unless permitted or required by law or regulation. You may
give permission to another person ("Authorized Person"), such as a family member, to access and manage your PHR or Benefit
Statement and such access shall be granted upon designation by you. Your permissions to access your PHR or Benefit Statement
may be terminated or revised at any time by changing the permission granted in your "Personal Profile". You may choose to
give the Authorized Person full access to your record, or you may choose to limit the information they can see. It is your
responsibility to confirm any information in your PHR that you permit an Authorized Person to enter.
When you terminate your authorization, that person will no longer be able to access your PHR or Benefit Statement;
however, they may have already seen or copied the information in your PHR and Benefit Statement. As a result, your
termination will not be effective for any such information that might already have been accessed.
Program Enrollment:
If you access online enrollment in programs such as a disease management program or Healthy Families Program or create a healthy
living plan, we will use the information to provide you with care management services such as coaching and other counseling,
and share your enrollment information and other provided information with BCBSA. For example, in the Healthy Families Program
we may collect the following types of information from parents or guardians about their child: child's name, age, gender,
height, weight, BMI, answers to the scorecard quiz. We may associate that information with one or more of the following
